At healthAbility we understand the added value that working in partnership delivers. We believe a broad range of partnerships that cross sector boundaries is essential to achieving the goals of our Strategic Plan and enabling us to best respond to our community needs.

Importantly, as demand for health services increases, there is an ever-growing need to work more efficiently and effectively with available resources. healthAbility already works extensively in partnership with others who have shared passion and values and understand the importance of having alignment of purpose for each partnership endeavour.

We are already well recognised for our ability to create meaningful partnerships with excellent outcomes through our integrated services such as IDEAS (Integrated Diabetes Education and Assessment Service), healthAbility Men's Shed, After Hours Mental Health Nursing Service and primary prevention initiative Baby Makes 3. These positive example shows that strategic partnerships have the capacity to translate learnings from demonstration projects into models of shared care across a region or policy and practice change on a national scale.

Our staff take pride in their ability to gather and work effectively with colleagues to achieve best results for the partnership, and importantly for the broader community. We believe that healthAbility can and will achieve our longer term goals for service growth and development by leading and partnering with others, by becoming a smarter health partner.

Connect4Health

Connect4Health was established in 2014 by Carrington Health, Inner East Community Health Service, Manningham Community Health and Link Health and Community. In an environment that has been dynamic, it now consists of the successor organisations healthAbility and Access Health and Community (AccessHC). Connect4Health recognises the potential to achieve better outcomes by acting in partnership, rather than alone and has facilitated more than $12 million in new services since 2014.

healthAbility and AccessHC each provide community health services across the Eastern part of Melbourne. While we continue to provide care and support to meet the needs of our own communities, we concurrently recognise the potential for greater levels of collaboration to achieve better health outcomes and value.

Connect4Health recognises the potential to achieve better outcomes by acting in partnership, rather than alone. Our purpose is to act collectively to deliver universal community-based primary healthcare and support, with a special responsibility for the vulnerable.

A more collaborative approach to primary health service delivery is required to meet the changing needs of our communities and respond to the challenges of a dynamic reform agenda across the health system. Eastern Melbourne Primary Health Network (Eastern Melbourne PHN)

Primary Health Networks (PHNs) have been established with the key objective of increasing the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and improving coordination of care to ensure patients receive the right care in the right place at the right time.

There are two collaboratives that the Eastern Melbourne PHN are involved in and healthAbility, with a catchment that traverse both the East and North East of Melbourne. The groups aim to improve people’s experience of health care services by reducing fragmentation and duplication, create efficiencies, listen to consumer voices, encourage innovation improve data and information flow and ultimately improve a person’s health care journey.

  • Eastern Melbourne Health Alliance (EMHA)
  • EMHA is a collaborative of health providers that deliver services in the catchment defined as the Local Government Areas (LGAs) Boroondara, Knox, Manningham, Maroondah, Whitehorse, Yarra Ranges.

Partners include healthAbility, Access Health & Community, Department of Health (Eastern Division), Eastern Health, EACH, Inspiro, Eastern Melbourne PHN.

  • Better Health North East Melbourne (BHNEM)
  • BHNEM is a collaborative of health providers that deliver services in the catchment defined as the Local Government Areas (LGAs) of Banyule, Darebin and Nillumbik. The people and communities in this catchment are the focus for BHNEM.

Partners include healthAbility, Austin Health, Banyule Community Health, Department of Health (North Division), Your Community Health, Eastern Melbourne PHN and North Western PHN.
