A program for effective self-management in cancer within a community health setting.
“Cancer is a major cause of illness in Australia with over 1 million Australians living with or who have lived with cancer."
Cancer survival rates are improving rapidly due to advances in treatment and earlier detection. Co-ordinated, integrated access to allied health professionals and self-management support interventions, underpins quality survivorship care.
With experience and expertise in the delivery of chronic disease management programs, community health services are well placed to bring together acute/sub-acute cancer interventions with ongoing community-based support best suited to survivorship care.
In this forum for health professionals hosted by our ‘Good Life – Cancer Survivorship’ project team, speakers will discuss the recent evaluation of the ‘Good Life – Cancer Survivorship’ referral pathway and share their experiences working within this pathway.
