It can be confusing trying to understand what services are available, funding options and how these can be accessed. Therefore, we offer a range of ways to help connect you to the services you need.

Service Connection

We have a team of Service Connectors who support older people unable to arrange aged care and other services without intensive support and don't have family or friends who can help.

Service Connectors can help people understand what aged care services and other services are available, set up an assessment and find and choose services. They can help with both accessing services for the first time and changing or finding new services and supports.

Access and Support Service

The Access and Support Service is free and provides short term support for frail older people, people with dementia, younger people with a disability (not eligible for the National Disability Insurance Scheme), and their carers, who need help to stay living at home.

The service assists you and your carer to understand the range of support services that may be available to you and how to access them.

Health Access Team

The Health Access Team is the first point of contact for information and referrals to a wide range of health and wellbeing services provided by healthAbility.

Our friendly Health Access Team are experienced health professionals trained to provide you with information about our services and other options that are available to you.

Home Care Package Management

Our experienced Home Care Packages care management team considers your individual situation and matches you with the care services you need.

A personalised care plan is developed together with you and anyone you would like to include in the management of your care at home.

You can use our in-house accredited care services, or if you already have a provider of choice, we can arrange the services directly.

With our personalised and flexible care management service you will always be able to change your arrangements, for whatever reason. You are always in control of the services you receive and who provides them.

NDIS Support Coordination

NDIS Support Coordination assists you to get your NDIS plan started and helps you to understand and manage all the NDIS supports available to you.

Support Coordination may be included in your plan as a stand-alone, capacity-building support.

A Support Coordinator will meet with you to explain your NDIS plan and help you to understand what needs to be done to access the supports you need.

They will also help you find the service providers that are best suited to you.

Some of the ways this is done is by linking you to personal care and community access workers, navigating housing, or assisting with daily activities, finding a job or finding an allied health therapist.
