Eastern Melbourne PHN announces a new network of care finders including healthAbility

We’re proud to be part of the network of service providers to help members of our community who would otherwise struggle to navigate the Aged Care system and other community supports. We are partnering with Access Health and Community to provide Service Connection in our local communities.
We look forward to working closely with Eastern Melbourne PHN, Access Health and Community, Care Connect, DPV Health, Migrant Information Centre, Villa Maria Catholic Homes, The Salvation Army, Merri Outreach Support Service, Wintringham and Housing for the Aged Action Group to establish this new care finder initiative, developed in response to the Royal Commission into Aged Care Quality and Safety.
