Support at Home

As of 1 November 2025, the Home Care Packages program has officially been replaced by the Support at Home program. Designed by the Australian government, this new program is intended to help older Australians continue to live safely, independently, and comfortably at home. The Support at Home program is a comprehensive and flexible home aged care solution, offering more aged care support at home to ensure quality care for those who need it most.

Our team of care partners can discuss your needs and living situation to develop a comprehensive care plan that may include personal care, home modifications, and extra support services. Whether you need help with daily tasks, specialised care, or home modifications, our team is here to help ensure that your needs are met.

With nearly 50 years as a trusted Community Health provider, our experienced team understands your care needs. We can help you to access wrap-around support, to help you remain independent in your own home and in the community.

Step-by-step guide to accessing aged care services in Melbourne

Step 1: Contact My Aged Care to find out what support services are available to help you stay in your home. Phone 1800 200 422.

Step 2: Think about the support you might need, such as personal care, help with household tasks, or support with social activities. We can help you plan for your assessment.

Step 3: You will be assessed by the Aged Care Single Assessment Service to confirm the level of support you may need.

Step 4: You will receive a letter from My Aged Care about your eligibility for the Support at Home program. Call us, and we will guide you through the next steps.

Step 5: You'll receive a second letter to confirm your enrolment in the Support at Home program.

Step 6: You have 56 days to choose a provider. As soon as possible, contact our My Aged Care team at 9430 9100 to discuss care management and any additional services.

Our Support at Home program in Melbourne

We aim to provide the best aged care services to older Australians and those living with disabilities or special care needs. The Support at Home program offers a range of services designed to meet basic support needs as well as complex care requirements.

These services include:

  • Personal care: Assistance with daily activities such as bathing, dressing, and grooming.

  • Home modifications: Ensuring your home is safe and accessible, with adjustments like ramps, grab rails, and more.

  • Health therapies: Including physiotherapy, speech pathology, podiatry, and other therapies to help with mobility and general health.

  • Community access and social activity groups: Ensuring you stay socially connected by arranging visits, transport to appointments, and community activities.

  • Transport: Helping you get to appointments and social events.
