Client Experience Survey – Prize Draw Terms and Conditions (updated July 2025)
Terms and Conditions of Entry
1. Participation in the Promotion constitutes acceptance of these Terms and Conditions. This Promotion is held at healthAbility Box Hill, Eltham, Wellington Road and Community Shed and at sites healthAbility provides services at. The Promoter is Nillumbik Community Health Service Ltd (ABN 32 180 310 839) trading as healthAbility.
2. The Promotion will be offered across two periods:
Period 1: Commences on 1 July 2025 and ends 31 December 2025.
Period 2: Commences on 1 January 2026 and ends 30 June 2026.
3. To enter the Promotion, entrants must complete a Client Experience Survey form by answering all fields asked in the survey.
4. This promotion is restricted to persons over 18 years old.
5. Participants must have engaged in a healthAbility service. Entry is limited to one per person, per day.
6. The winner will be selected at random from the valid entries (as specified in point 3).
7. The prize/s are: 5 x $100 digital gift vouchers (Visa or Mastercard) valued at $100 each. These gift vouchers cannot be redeemed for cash.
8. The Prize/s will be drawn at healthAbility Box Hill offices – Level 3, 43 Carrington Rd, Box Hill VIC 3128 on the 1st of January 2026 and 1st of July 2026, or at the earliest convenience of the Promoter.
9. The Winner will be notified via phone or email.
10. The prize will be emailed to an email address confirmed by the winner.
11. The Promoter and the Centre reserve the right to request Winners provide suitable identification of proof of age and identity prior to awarding a Prize.
12. The Promoter reserves the right to announce and/or publish the Winners name and/or photograph for publicity purposes.
13. The draw for an unclaimed prize may take place on 1 February 2026 and 1 August 2026.
14. Entries that are illegible, incomplete or do not comply with these Terms are invalid.
15. The Promoter reserves the right to verify entries and claims for Prizes and reserves the right to disqualify an entrant where the entry process has been tampered with.
16. The Promoter is not responsible if the value of the Prizes changes between the date of publication of these Terms and the date the Prizes are awarded.
17. The Promoter may at its absolute discretion, suspend, cancel or recommence the Promotion if (a) for any reason the Promotion is not capable of proceeding as planned; (b) if the Promotion is corrupted or the administration, security, fairness, integrity or proper conduct of the Promotion is affected; or (c) if the determination of the winner or delivery of prize is prevented or hindered by any event outside the Promoter’s control.
18. Except for any liability not excluded by law, entrants release and indemnify the Promoter from and against all actions, claims and liabilities, that the entrant now has or in the future may have against the Promoter for any loss, damage or injury (including any special or consequential loss) suffered as a direct or indirect result of the entrant’s participation in the Promotion.
19. The Promoter’s decision in relation to this Promotion is final and binding on entrants and the Promoter will not enter into any correspondence.
20. By entering the Promotion, entrants consent to their personal information being used by the Promoter for the purpose of conducting this Promotion and notifying the Winner. All personal details of the entrant will be stored at the offices of the Promoter. A request to access, update or correct any information should be directed to that office. The Promoter’s Privacy Policy may be obtained at Privacy and Confidentiality Policy (healthability.org.au)
21. The promoter will remove all entrant personally identifiable information (PII ) from healthAbility systems once all prize winners have received their prizes.
22. Officers, employees and immediate families of officers and employees of the Promoter and their related bodies corporate and contractors and any other organisations associated with this Promotion are ineligible to enter.
