The Access and Support Service is free and provides short term support for frail older people, people with dementia, younger people with a disability (not eligible for the National Disability Insurance Scheme), and their carers, who need help to stay living at home.

The service assists you and your carer to understand the range of support services that may be available to you and how to access them.

The Access and Support Service is available to people living in the cities of Boroondara, Manningham, Monash and Whitehorse.

Access and Support Service is funded in Victoria by the Australian Government Department of Health and the Victorian Government Department of Health and Human Services.

Meet David Well

David is 66 and has arthritis in his knee. As David is getting older, he is finding that it is harder to keep on top of his day to day chores and his mobility is declining.

Watch how David was referred to our Access and Support service, who help him apply to My Aged Care for an assessment. They also arrange for him to see a clinician to assess his arthritis.
