Living with a long-term health condition can be challenging. You may need to manage appointments, treatments, services and lifestyle changes all at once. The Care Coach program is a healthAbility pilot that provides short-term support to help you better manage your health and achieve goals that matter to you. 

Our Care Coach team works with you to understand your needs, coordinate your care and connect you with the right services and supports. The program compliments the care you already receive from your GP, specialists and other healthcare providers.

Who is the program for?

The Care Coach program may be suitable for adults who have been diagnosed by a GP or specialist with:

  • Type 2 diabetes
  • Cardiovascular disease, including:
    • heart disease
    • stroke
    • high cardiovascular risk

The program may benefit people who experience stress, anxiety, depression or other wellbeing challenges alongside their physical health condition.

How can the program support you?

Support is tailored to your individual needs and goals, and may include:

  • Coordinating care between your healthcare services and providers
  • Health coaching and support to manage your condition
  • Help setting and achieving your health goals
  • Access to allied health and counselling services as part of your care plan
  • Support from people with lived experience
  • Individual appointments, phone calls, video appointments and/or group sessions
  • Education, digital tools and self-management resources

How long does the program run?

Most participants receive support for around three to six months. The level of support will depend on your individual needs and goals.


Cost

There is no additional cost to join the Care Coach program.

Appointments with health practitioners will continue under your existing funding arrangements. If you currently pay a co-payment or fee for appointments, these costs will continue to apply.
