To enable people to live as independently and healthily as possible, we understand the need for you to get to places outside of the home and stay connected to the community.

We have a fleet of vehicles and drivers that provide transport to and from home, to and from our centre-based Connect Well Groups and to and from personal or community-based activities.

In Home and Community Support services

healthAbility In Home services are available for both private and publicly funded clients. All of our carers have completed mandatory checks to ensure your safety and that your care needs are met. We want you to be comfortable with the person who enters your home, so we will match you with a support worker who shares similar values, background, language and culture.

We offer:

  • Connect Well Groups
  • In Community Assist
  • Domestic assistance
  • Day respite care
  • Home maintenance
  • Gardening
  • Community Access (transport)
  • Meal preparation
  • Personal care

Live well and independently

If you or a loved one needs support living at home, we can help you navigate the My Aged Care (MAC) system, coordinate the full spectrum of care needed, and ensure the best possible value from your Home Care Package.

Our professional and supportive team work together with you and those important to you, to give confidence in decision making and in our safe and trusted services.

With extensive experience in aged care, a broad range of services and an intimate knowledge of resources in the local community, we can adapt services as your needs change.

With locations in Melbourne’s East and North -East regions, we offer flexible service delivery options either at one of our sites, in your home or community setting, or via telehealth.
