Terms and Conditions of Entry
1. Participation in the PWI Survey constitutes acceptance of these Terms. This survey is held at healthAbility Box Hill and Eltham service sites. The survey is from healthAbility trading as Nillumbik Community Health Service Ltd.
2. To enter the competition, clients must complete the second version of the PWI survey. This survey is sent 6 months after clients have completed the first PWI survey.
4. One entry per person only.
5. The winner will be selected at random from the valid entries.
6. The prize is: Five x $100 vouchers and cannot be redeemed for cash. Two $100 vouchers will be drawn on 1st August 2025 and three $100 vouchers will be drawn on 31st October 2025
7. The Prize will be drawn at healthAbility Box Hill – 43 Carrington Rd, Box Hill VIC 3128.
8. The Winner will be notified via phone or email.
9. The prize must be collected from either healthAbility Box Hill or Eltham sites, during centre operating hours.
10. healthAbility reserves the right to request Winners provide suitable identification of proof of age and identity prior to awarding a Prize.
11. healthAbility reserves the right to announce and/or publish the Winners name and/or photograph for publicity purposes.
12. The draw for an unclaimed prize may take place on 1st December 2025.
13. Entries that are illegible, incomplete or do not comply with these Terms are invalid.
14. healthAbility reserves the right to verify entries and claims for Prizes and reserves the right to disqualify an entrant where the entry process has been tampered with.
15. healthAbility may at its absolute discretion, suspend, cancel or recommence the Competition if (a) for any reason the Competition is not capable of proceeding as planned; (b) if the Competition is corrupted or the administration, security, fairness, integrity or proper conduct of the Competition is affected; or (c) if the determination of the winner or delivery of prize is prevented or hindered by any event outside of healthAbility's control.
16. Except for any liability not excluded by law, entrants release and indemnify healthAbility from and against all actions, claims and liabilities, that the entrant now has or in the future may have against healthAbility for any loss, damage or injury (including any special or consequential loss) suffered as a direct or indirect result of the entrant’s participation in the Competition.
17. healthAbility’s decision in relation to this Competition is final and binding on entrants and healthAbility will not enter into any correspondence.
18. By entering the competition, entrants consent to their personal information being used by the centre for the purpose of conducting this Competition and notifying the Winner. All personal details of the entrant will be stored at the offices of healthAbility. A request to access, update or correct any information should be directed to that office. The Promoter’s Privacy Policy may be obtained at Privacy and Confidentiality Policy (healthability.org.au)
20. Officers, employees and immediate families of officers and employees of the Centre and their related bodies corporate and contractors and any other organisations associated with this competition are ineligible to enter.
