healthAbility and Eastern Health present Integrated Diabetes Education Assessment Service (IDEAS) at the Victorian Health Sector Summit

We were excited and honoured to be asked to present at last week’s Department of Health Victoria Health Sector Summit hosted on 30 and 31 March.

This summit will bring together nearly 500 representatives of Victoria’s health sector with the purpose of developing a ‘2033 Victorian vision of change in health and care’.

In partnership with Eastern Health, Agata Jarbin – healthAbility’s CEO and Professor Chris Gilfillan – Clinical Director, Endocrinology, Eastern Health, presented the Integrated Diabetes Education Assessment Service (IDEAS) program. IDEAS provides people with Type 2 Diabetes a complete wrap-around service in a community setting to prevent and manage any of their health complications. The service is supported by a range of health professionals including an Endocrinologist and provides practical education and support to empower participants in managing their health and well-being for the long term.

They presented at the Health and Care Outside the Hospital session which focused on the reduction of preventable hospital admissions and to ensure that all Victorians have access to chronic disease management and preventative care services.
