Suitable for: Adults who have not been assessed before or those requiring re-assessment after 12 months or have experienced a change in mobility
Service includes:
- Comprehensive 60-minute in-home assessment – We evaluate daily function, home safety, and set personalised goals
- Personalised recommendations – Simple solutions to improve comfort and mobility. Our formal assessment report will include recommendations
- Guidance for small aids e.g. prescription of small aids and provision of supplier and installation recommendations.
- Basic home modification advice – Recommendations for rails in the shower, toilet, or entry to enhance safety.

Package price:
$645 inc GST and travel time (1 hour return)
Minimum 3 hours of time required
Important service information:
- Ongoing follow up, complex home modifications (ramps, bathroom renovations etc.) will require additional time. Quote estimates can be provided.
- The initial home visit is subject to successful completion of a home visit risk assessment.
- Travel time - Our gift card packages include 1 hour total travel time from our offices (Box Hill/ Eltham) , if additional travel time is required, additional fees ($82 per additional hour) may apply. View service area.
- Personal information - By purchasing this gift card, you confirm that you have the recipient’s permission to provide their personal details. These details will be used to create a provisional client record, which will be verified at time of booking or service delivery.
- Cancellation/Reschedules - If for any reason our team member is unavailable, we will contact you to arrange an alternative day/time. If you are not available for a scheduled service, please cancel by calling us 03 9430 9100. If services are cancelled less than 24 hours from your scheduled service, you will be charged for the service.
