Our Heart Health services support people in managing cardiac conditions, including heart failure and other chronic medical conditions through a combination of one-on-one support and group programs.

A physiotherapist, a nurse and an allied health professional facilitate this program. This supervised program that builds strength and endurance and helps maintain function and independence. 

The program supports many areas of an individual's health and well-being, including:

  • Physical activity: To assist you with improvement in your physical and psychological well-being through a team approach
  • Health education: To provide an education program to both you and your family that helps you to understand your health needs and ongoing management
  • Strategies to help modify behaviour: To provide a graduated exercise program that assists you in becoming physically active at a level compatible with the function of your heart
  • Long-term support: To encourage you to become an active participant in the management of your health and well being
