The Home and Community Care Program for Younger People (HACC PYP) supports people 65 years and under, and Aboriginal and Torres Strait Islander people 50 years and under, if their capacity for independent living is at risk. 

This may be due to chronic illness, mental health issues, disability or other conditions where they need one-off, intermittent or ongoing support to undertake the activities of daily living.

The aim of HACC PYP is to optimise people's health and wellbeing, and enable them to live safely, independently and maintain social and community connections.

You may be eligible for this funding if you have a disability, injury or chronic health issue, and are:

  • Under 65 years old
  • Under 50 years old if you are Aboriginal or Torres Strait Islander
  • And do not have NDIS (National Disability Insurance Scheme) funding
